Please report a broken or redirected official link, changed fee, changed renewal date, new continuing-education requirement, incorrect authority, wrong credential route, inaccurate contact information, accessibility barrier, or misleading wording.
What to include
Provide the NursingLicenseBoards.com page URL, a concise description of the problem, and the official source supporting the change. Do not send Social Security numbers, application numbers, portal passwords, payment information, patient records, fingerprints, or confidential licensing files.
Review process
A submission is stored in a protected server directory for editorial review. It is not automatically published. We compare the report with the responsible authority, record a material correction in the update log, and update the task audit where appropriate.
Urgent licensing matters
Do not use this form for an emergency, a formal complaint against a licensee, an appeal, a disciplinary response, or a deadline extension. Contact the responsible licensing authority directly.
